When your passport is renewed, your valid Thai visa, extension and re-entry permit remain in the old passport. They must be formally transferred to the new passport at immigration before you travel or file your next report.
Your embassy must usually provide a letter confirming the passport replacement, and translations may be required.

Counsel's note
Travelling on a new passport without completing the transfer may cause your extension to be treated as missing on exit.
